Easy pre-registration with our online form!

Please fill out one form for each child you wish to enroll. 
We will contact you within 1 business day to confirm receipt of registration. 
Enrollment is complete once the enrollment fee or deposit has been received.  Payment can be mailed or made in person.  Payment can be made by cash or check payable to SW Dance.  Cards are not currently accepted.  (Please do not mail cash.)
